Kerrington’s Heart Focus Group Application
The purpose of this focus group is to obtain a deeper insight and diverse, broader perspective related to a wide range of topics related to Kerrington’s Heart.
General Guidelines and Values for Kerrington’s Heart Focus Group
Compassion: Our compassionate aid will allow us to work alongside Kerrington’s Heart, a well-know organization in Kentucky who supports CHD families across the state. This focus group will not be used to talk negatively about any other group, hospital, or medical staff.
Trust: We will build trusting relationships with each other. What we talk about and discuss will remain confidential between the group.
Accessibility & Accountability: We will meet once a month using Zoom for approximately 60 minutes. Prior to meetings, you may be asked to individually work on assignments and be ready to discuss as a group. We will hold ourselves accountable to help Kerrington’s Heart be the best that it can be by serving and connecting with the CHD community while learning what its needs are.
2. Time Commitment
Kerrington’s Heart requests support team members commit for 1 year baring any medical emergencies.
3. Kerrington’s Heart Focus Group Confidentiality Policy
Kerrington’s Heart Focus Group leadership will hold as confidential all disclosures made in the context of the group.
You must maintain the confidentiality of information that is discussed within the group.
All statements, presentations, and minutes are confidential information.
You must be mindful of conversations that you have in public spaces to protect other members of the group and their families.
Everything related to the focus group will not be put on social media.
